WHO IS ELIGIBLE FOR TREATMENT?
Any employee of the Platte County and Smithville School districts, along with their spouse and dependent children are welcome regardless of their insurance coverage.​
AT WHAT AGE ARE DEPENDENTS SEEN?
Our Family Nurse Practitioner may start seeing your child at age 6 months.​
WHAT IF I DON'T CARRY THE DISTRICT'S INSURANCE? CAN I STILL TAKE ADVANTAGE OF THE WELLNESS CENTER?
Yes, as an employee, you are eligible to take advantage of all the Wellness Center benefits such as flu shots, sports physicals, sick visits, wellness coaching and more!
​HOW MUCH DOES IT COST, AND DO I PAY WHILE I'M THERE?
Depending on your eligibility, there may be no charge for the visit but never more than $40 for your treatment. We accept cash, check or credit card. Your H.S.A. debit card is welcome also.​
WHAT SHOULD I EXPECT WHEN I ARRIVE?
If it’s your first visit to the Wellness Center, there will be some registration to be completed. But once you’re in our system, your wait time will be minimal.
​CAN I SPEED UP THE REGISTRATION PROCESS?
Yes, and we encourage you to do so. Click HERE for the documents you will be required to complete. Please download and complete the forms. Then, bring the completed forms with you to the appointment.
​IF I NEED A PRESCRIPTION, HOW WILL THAT BE HANDLED?
We can send your prescription to the pharmacy of your choice.
